CLIN FEATRS IN PED ED

presenting complaints

illness is different from previous illnesses


child is less active

child is sleepy

 child is hard to wake up

child cries a lot

 child has abnormal behaviour

child's speech is inconsistent

 fever present?

 HISTORY- duration of fever number of days

 fever not improved with antipyretics

diarrhoea

bloody diarrhoea

 stomach ache

 vomiting

 persistent vomiting

 bile-stained vomiting

does your child have reduced intake?

 does your child pee less?

short of breath

coughing

headache

 neck pain


 observation

clinician concern+

 clinical impression

child is seriously ill

 child is irritable

 child is drowsy

 child had reduced consciousness

 child is inconsolable

child is moaning

 child has nasal flaring

 chest wall retractions

child laughs less

clinical examination

highest temperature (measured or reported) temperature in °C

 pus on tonsils

 signs of acute otitis media

 bilateral otitis media

discharging ears

 extensive adenopathy

 redness and or swelling of face  purulent conjunctivae
 bilateral purulent conjunctivae

 dyspnoea

 crackling

 reduced breathing sounds

 rhonchi

 cyanosis

 peritoneal irritation

 petechial rash

 meningeal irritation

 reduced peripheral circulation

 pale

 skin turgor

 fontanelle tension normal/bulged/sunken

 swollen limb, non-weight bearing extremity

 breathing rate times per min

 heart rate times per min

oxygen saturation percentage

 capillary refill number of seconds  diagnosis working diagnosis string variable
