area that has been identified that needs improvement in the identification and
management of a sick child is communication within the Multi – Disciplinary
Team.
•This
may be information not being handed over, e.g. blood tests not being taken,
results not being looked at, physiological observations outside of normal range
not being reported or acted upon, frequency of observations not being handed
over.
