“When carbohydrate is omitted from the diet in a person that has been in a high carbohydrate diet for a long time, the first effect is that the digestive system and the liver will run out of glucose. To maintain the desired blood sugar level around 83 mg/dl, the pancreas will stop creating insulin and will start to create glucagon, a hormone that starts the process of obtaining glucose from stored glycogen. It takes about three days for the glycogen stores of a person to be depleted. During this time, the body will still be a carbohydrate-burning metabolism. Blood sugar is constant. The depletion of glycogen results in a rapid weight loss because a great quantity of water is released.”

Tirobhava is one of the five cosmic actions of Lord Shiva, which is believed to be performed in his form as Shiva Nataraja. Shaiva Puranas describe at length the dance of Shiva Nataraja, which embodies pacha kriyas or the following five cosmic actions: Shrishti (creation), Sthiti (preservation), samhara or laya (destruction), tirobhava (unveiling of illusion), and granting of anugraha (grace of salvation).

The act of tirobhava is symbolized by Shiva Nataraja’s right foot, which is shown as apasmara purusha (trampling a demon) who is the symbol of evil and illusion.

Steroids modify the natural history of croup  – they give rise to clinical improvement within 30 minutes, and decrease the need for hospitalisation, the duration of hospitalisation and the need for intubation. Oral steroids (dexamethasone 0.15–0.6 mg/kg or prednisolone 1–2 mg/kg) are the treatment of choice, but if the child will not take oral medication or is vomiting, then nebulised budesonide 2 mg can be used. Dexamethasone can be repeated after 24 hours if clinically indicated; budesonide aerosol can be given more regularly (every 30–60 minutes) based on clinical response
