If the clinician
makes a diagnosis that later turns out to
be wrong, the question will inevitably be
asked, ‘At the time why wasn’t it clear that
the diagnosis was X?’ Notwithstanding
gross negligence there are four factors
(physiology, communication, heuristics
and external factors) that complicate the
process of diagnosis.
