At its most basic, radical acceptance is a practice that involves accepting emotions, thoughts and circumstances that are unchangeable and out of your control. To radically accept means to completely recognize and accept the reality you’re in—even when that reality includes pain or discomfort. Regularly practicing radical acceptance can help us cope with painful emotions so we can move on or even come up with a plan to make an uncomfortable situation better.

Radical acceptance boils down to preventing our pain from turning into suffering, adds Ernesto Lira de la Rosa Ph.D., a licensed clinical psychologist in New York City, Hope for Depression Research Foundation media advisor and clinical assistant professor at New York University. “When we experience something painful, we may want to avoid the discomfort, ignore it or push it away.” Instead, radical acceptance is rooted in the belief that when we observe unwanted experiences rather than reject them, we may not feel the same level of frustration, shame or sadness we may have without turning to this practice.

While the theory of radical acceptance originates from Buddhist teachings, the term was first coined in 1993 by Marsha Linehan—the same psychologist who developed dialectical behavior therapy (DBT), a form of talk therapy. DBT helps people learn and use mindfulness and emotional-regulation strategies—one of which is radical acceptance. “DBT is designed to help people be present, regulate emotions, develop coping skills and improve relationships,” adds Minor.

But what causes cancer to spread? We don’t really know, and we are unlikely to find out anytime soon because only about 5 to 8 percent of US cancer research funding goes to the study of metastasis. Our ability to detect cancer metastasis is also very poor, although I do believe we are on the verge of some key breakthroughs in cancer screening, as we’ll discuss later. Most of our energy has been focused on treating metastatic cancer, which is an extremely difficult problem. Once cancer has spread, the entire game changes: we need to treat it systemically rather than locally.

 Our ancestors did not need to know that a bear-representation was currently active in their brains or that they were currently attending to an internal state representing a slowly approaching wolf. Thus neither image required them to burn precious sugar. All they needed to know was “Bear over there!” or “Wolf approaching from the left!” Knowing that all of this was just a model of the world and of the Now was not necessary for survival. This additional kind of knowledge would have required the formation of what philosophers call metarepresentations, or images about other images, thoughts about thoughts. It would have required additional hardware in the brain and more fuel.

Although the term radical acceptance may sound like someone is being instructed to act apathetically or simply just accept all situations—including negative ones—the aim is to embrace facts and acknowledge pain rather than fight it. “[Radical acceptance] does not mean that we are being passive or approving of what is happening,” says Dr. Lira de la Rosa, “rather, it means we are radically accepting that we are in a position where we feel pain and we can honor and acknowledge this pain.”

Whether you opt for a dedicated radical acceptance journal or a scrap of paper, Dr. Lira de la Rosa says it can be helpful to make a list of all the behaviors you would engage in if you did accept your current reality. “Then,” he says, “when you are experiencing a negative emotion, such as sadness, shame or guilt, write about what you are feeling and invite these emotions to run their course.”

However, a systematic review concluded that the studies reviewed were not of good enough quality nor long enough duration to be reliable[1].
