Be suspicious in children that are colicky, have palpitations, lightheadedness, syncope, seizure, or exertional symptoms. These children may have an underlying cardiac cause to their chest pain.

In selected children with chest pain who also had abnormal EKG findings or a fever, obtaining elevated troponin had diagnostic utility for diagnosing myopericarditis. In addition, troponin may be useful in children with chest pain associated with chest trauma, overdose, poisoning, or significant cardiac pathology
